Cosmetic vs. Functional Eyelid Surgery
Patients undergo eyelid surgery to address cosmetic or functional needs (or in some cases both). Cosmetic blepharoplasty is done to improve a person’s appearance, reversing signs of aging like sagging skin and fine wrinkles. This results in a more youthful and revitalized look.
Functional blepharoplasty is performed for medical reasons. When drooping upper eyelids impair a person’s vision, loose skin is removed during functional eyelid surgery to lift the eyelids, resolving obscured vision.

Upper and Lower Eyelid Surgery
There are two main types of eyelid surgery: upper blepharoplasty and lower blepharoplasty.
Upper eyelid surgery
Used to get rid of sagging skin, fine wrinkles, excess fat, and puffiness on the upper eyelids. This procedure can address upper eyelids that obscure a person’s field of vision. It involves a small incision in the natural crease at the top of the lid.
Lower eyelid surgery
Used to get rid of puffy bags under the eyes or sagging lower eyelid skin. When excess fat and skin are being removed, an incision called the transcutaneous incision is made at the lash line on the lower lid. When only excess fat is being removed/repositioned, an incision called the transconjunctival incision can be made inside the lid instead.
Ptosis Repair
Eyelid ptosis (sagging) is a condition that occurs when weakened levator muscles are not able to lift the upper eyelids properly. This results in drooping lids, which obscure a person’s vision. It can come about due to hereditary, congenital, or muscular-neurological disorders.
Ptosis repair is a type of eyelid surgery performed to shorten or tighten the muscle in order to ensure that it can properly open and lift the upper eyelid. It is a purely functional procedure. The details of this procedure depend on the severity and cause of the ptosis.
